Birth and Death Certificates

Are you looking for a birth or death certificate? Simply download an application for a Certified Copy of a Birth or Death Certificate, fill out the application and bring it with your photo ID and your signature to the Heath Department or *mail it to us at 55 South 500 East, Heber City, Utah, 84032.  *If mailing, a copy of the front and back of your photo ID is required.

Birth Certificates:

Birth certificates are available if you were born in 1943 - 1974, or 1951 to the present.  Additional years back to 1905 are available for an additional expedite fee of $12.00.

 

Actas de nacimiento están disponibles si usted nació en los años de 1945, 1946, o 1951 al presente. Los años adicionales hasta 1905 están disponibles por una cantidad adicional de $12.00.

 

Death Certificates:

Death certificates are available if the death occured from 1995 to the present. Additional years back to 1905 are available for an additional expedite fee of $12.00.
 

 

Additonal Copies:

Additional Copies of Birth or Death Certificates are $8.00

We accept personal checks, Visa, MasterCharge, money orders, and cash

 

Forms of Identification

List of acceptable forms of Idenfiticatin. Some document require two forms of ID. Be sure to review this list for acceptable ID.
